Join the Integra Community Care Network

When you’re a part of Integra, you’re a part of a team of social workers, community health workers, certified diabetes educators, and other health care professionals. If your patient is an Integra member and needs access to additional services that can help them stay healthy, Integra is here to support them.

We can help Integra members set up transportation to medical appointments, connect them to community resources, such as food and housing services, provide health education to help manage chronic conditions like diabetes, asthma, and COPD.

Share In Savings

Our ACO is a partnership of primary care physicians, specialists, hospitals and others who work together to provide efficient healthcare delivery, meet quality-of-care objectives and, in turn, share in savings.

Robust Health Support Services

By providing a robust platform of population health support services and infrastructure, Integra is committed to supporting community-based physicians who provide exceptional and efficient patient care. 

Incentivized Care

Unlike traditional fee-for-service payment systems that reward physicians on the number of procedures performed, Integra incentivizes our primary care physicians to keep their patients healthy through prevention and avoidance of unnecessary hospitalizations, emergency room visits, tests, and procedures.

I-SPII: Integra Social Partnerships Innovation Initiative

I-SPII was an initiative of Integra’s Medicaid Accountable Entity (AE) program to develop partnerships with Community-Based Organizations (CBOs) to plan and pilot interventions to address health-related social needs.
