As an Integra patient, you benefit from team-based care.
Team-based care within Integra means there’s an entire team of people working to keep you healthy. In addition to your primary care physician, Integra has a team of nurse care managers, social workers, community health workers, and pharmacists that communicate with each other and your doctor, to make sure you have everything you need to be healthy.
When all of your providers communicate with one another, they’ll have a more complete picture of your health and will be better able to treat you and keep you healthy.
In addition to the benefits of team-based care, you’ll have access to a number of programs.
We have a dedicated team of community health workers and nurse care managers who are available to assist you with all of your health care needs, whether it’s connecting you with community resources like housing and food services, providing health education to help manage chronic conditions like diabetes, hypertension, asthma, and COPD, we have everything you need to be healthy.
When you’re a part of Integra, you’re a part of a team of social workers, community health workers, certified diabetes educators, and other health care professionals. If your patient is an Integra member and needs access to additional services that can help them stay healthy, Integra is here to support them.
We can help Integra patients set up transportation to medical appointments, connect them to community resources, such as food and housing services, and provide health education to help manage chronic conditions like diabetes, asthma, and COPD.
